I'm setting up a small photo printing business and need to integrate an upload button with my online store, anyone have any ideas?

AS you will see I have created a form to allow the customer upload their photo but they then need to specify the format and size. It is possible on that page to do both but they are not linked/integrated so the customer has to use the back button and remember to add their images. I would love if it could be integrated - so you firstly upload the image - then you are prompted to go and complete your order. I hope this makes sense and thank you once again for taking the time to help.
