I would like my donors to have choices on where their money goes, ie:
Food
Medical
Infrastructure
Is this possible and where do I go to customize this?

In my opinion, your best bet to accept donations on your site is by creating a Paypal donate button from your Paypal business account, then place it on your site. You can set up the button directly through your Paypal account and during setup(end of step 3 in the paypal donate button setup), you can add specific programs that your donors can select from.
